Place your cut up chicken in the bottom of your crockpot.
Add broth, heavy cream, and dry wine (if using). Add tomatoes, minced garlic, salt, pepper, and Italian seasoning.
Stir to combine. Add cauliflower.
Cover and cook on low 4-5 hours.
Remove cauliflower with a slotted spoon and place in a blender. Add a little of the broth to the blender. Bled until the cauliflower is completely broken down.
Stir the cauliflower mixture back into the crockpot. Add the pasta and spinach. Stir and cook until the spinach and pasta are done to your liking.
Serve hot with parmesan cheese and red pepper flakes, if desired..
